I don't think this is related to java3d init routines, since in don't use any 
graphics and/or the xorg-driver-fglrx driver.

I did an upgrade today as well... [from dpkg.log: 2008-07-16 08:57:21
status installed libc6 2.7-10ubuntu3] and after rebooting my jboss AS
doesn't start anymore. I'm trying to figure out how to revert to a
previous version.

I would like to add that symptoms are consistent with previous posters -
network icon shows previous state, I'm unable to reconnect the adapter
and as far as I know only rebooting helps. Will try removing and
reattaching the adapter.

I can confirm the same bug in 11.10 - x86. 
(Linux htpc 3.0.0-14-generic #23-Ubuntu SMP)

Adapter fails with:
phy0 -> RT2x00usb_vendor_request: Error - vendor request 0x07 failed for offset 
0x308c with error -110

I can also confirm that it seems related to network load - I've always
had the card fail during intense downloading, but haven't noticed it if
no torrents were running.

I can confirm this problem occurred today after an upgrade to 2.6.35-23
from 2.6.35-22. My USB mouse stopped working, changed it for two
alternative mice, both failed to work. Some ports detected the mouse,
others didn't.

Failed on:
uname -a
x 2.6.35-23-generic #35-Ubuntu SMP Sat Oct 16 20:45:36 UTC 2010 x86_64 
GNU/Linux

Running on: HP 6730b laptop.

With same errors as before (c/p) (dmesg output):
[ 1235.301622] hub 2-0:1.0: unable to enumerate USB device on port 1
[ 1237.898293] hub 1-0:1.0: connect-debounce failed, port 3 disabled

lsusb fails to print out anything (hangs) if I connect any of the before
mentioned USB mice.

Resolved by switching back to -22-generic. Is there something wrong with
USB drivers in -23-generic?

Add to previous comment to clarify: Some ports detected the mouse (and
turned it on, visibly by powering on the LED), others didn't, but the
mouse didn't work on any port (failing with unable to enumerate USB).
Perhaps it is important to state that Logitech Mouse failed once and
stopped reconnecting to the system, while Razer BoomSlang gaming mouse
kept cycling and failing with a different error.

I'm getting somewhat similar crashes on my HP 6730b with Intel GMA
graphics. Crashes don't occur as often and I can safely say that they
happen on AC or battery power. I might go a couple of days
(realistically two - three max) without a hang. Besides problems with
random hangs, I do have problems with standby - sometimes computer
successfully goes into standby, sometimes it just hangs and waits there.
The same occurs sometimes when booting up the computer. Logs look
unproblematic, but they do share a common theme:

1. I'm getting the same problems when I disconnect my power cable (actually

I'd like to confirm that this bug is still very present for me - it was present 
in 10.11 and now in 11.04. On resume on suspend disk starts to heavily read 
from the hard drive. Two things to note:
1. This doesn't occur on every resume, but it does occur more often if I use 
the laptop for some time.
2. When it does it heavily reads from the hard drive for 3-30min. I usually get 
bored with the waiting and simply force shutdown the computer (using the 5s 
hold of the power button).

I tried following this tread, but no apparent fixes were provided for me
to try out.

Bug persists on Karmic Koala final (upgraded from beta to:
2.6.31-14-generic x86_64 SMP)

I tried:
aticonfig --initial
aticonfig --acpi-services=off

and I keep getting the black screen of death no matter what. 
xorg-driver-fglrx version: 2:8.660-0ubuntu4

If needed I can provide log files (even if they don't report any
errors).
