the monitors.xml is what is written by the gnome-display-properties
configuration dialog, what you described before seems to be that you
managed to have the tools writting screen settings which were not
working on your configuration, it would be interesting to know if those
worked and stopped due to changes in your settings or the driver or if
the configuration written was buggy

As of last December, with the version of ~/.config/monitors.xml attached
to comment #3 present, the problem occurred. With it absent, it did not.

Several updates to the kernel and other components have happened in the
intervening time. gnome-control-center is currently at version
1:2.28.1-0ubuntu1, and according to Update Manager all installed
packages are up to date. I currently have no monitors.xml file, and
don't know exactly how it came into existence.

I just tried restoring the bad monitors.xml file, and the observed
behavior is that I was able to log in, got a complete desktop at the
expected display resolution, but had no mouse cursor at all. Driving by
dead reckoning, I was able to poke at a few things to demonstrate that X
and the desktop seemed to be otherwise functional, and succeeded at
logging out again.

It seems plausible that there are some remaining issues with gnome-
control-center or some related component, but I don't know how to
directly reproduce the original issue.

I can confirm that changing display resolution seems to be a necessary trigger 
for the problem -- I'd originally left this detail out of my bug493016 report, 
but Bryce suggested they might be dups, and this report reminded me I'd tweaked 
resolution.
Again, only one monitor on this ancient box, so 2-display config isn't a 
necessary trigger.

There is one physical VGA connector on the card, and no other video
output such as a TV out connector, so I certainly didn't deliberately
configure more than one monitor. I am reasonably certain that this
specific CRT is also the only monitor that has been connected to this
box since Ubuntu was installed at all.

I had used the GNOME tools to change the display resolution, and been
puzzled by that setting not sticking across a power cycle. After
changing it a second time, it appeared to stick, and I stopped worrying
about it. Shortly after that an update to the kernel, gdm and the
screensaver (among other random things) was pushed and I had this
problem.

It doesn't look like the GNOME tools allow configuring a second monitor
if one isn't actually plugged in and available so it should be hard to
do by accident.

That file is actually provided by the gnome display configuration tools,
so moving this bug to gnome-control-center.

Perhaps your output names changed or something, and gnome-display-
properties is not handling the error that this causes.

I have found a workaround, and appear to have repaired my original user
account's settings. I found by comparing the various settings files and
folders that the bad user had a file named ~/.config/monitors.xml but
the newly created and usable user did not.

Renaming that file was sufficient to allow my bad user to log into GNOME
again.

I've attached the offending file so that further investigation can focus
on why this configuration file causes the GNOME session to not start
successfully. Aside from the fact that it appears to indicate that I
have two monitors of differing resolutions located on top of each other,
I'm not sure what is wrong. If the content is bad, then GNOME should
have rejected it and used some sensible fallback configuration. At
minimum, it should have logged the problem somewhere.

Interesting. A brand new user account gets a working session.

I created a new user, and tried logging in to the default GNOME session.
This gave me the default desktop, panels and menus. At this point, I can
see that even a "successful" login produces a fair number of error
messages in the .xsession_errors file. In particular, the message that
had seemed suspicious to me initially about an assertion failure in
gnome-settings-daemon is still there.

However, later messages in the failed session such as "Connection reset
by peer" and "Resource temporarily unavailable" referring to the X
server do not appear in the successful session.

I would like to figure out what significant difference there is between
the old and new accounts that causes the session to fail, both so I can
repair my original account, and so that whatever bug caused the settings
to be broken in the update can be repaired.
