Yes, I have one customer using the TSM 3.7 (ADSM in the past) on NT 4.0 SP5 with the same problem on QualStar TLS 4440 library(40 cartridges + 2 drives). The resolve was to fix TSM 3.7.0.0 to 3.7.3.0. In 3.7.3.0 configuration the server work fine without freezing and offline tapes.
