You'll need two dead end bails (eyestraps).
Suggestion, consider standing the jam cleats off the mast with Starboard or
teak pads. Why? So your crew can easily grab the up/down haul line. If
it's too close to the mast it will be hard to grab or your crew will scrape
their knuckles.

I just need 2 cheek blocks, 2 jam cleats and a fitting to dead end the control
line. I’ll post
Andrew, the item in the pic is very similar to what was installed originally
(and remains, working well) on my 33-2. There are other parts also as there is
a continuous line for controlling it. (Memory vague - I don’t think much about
it - it just works). I tried to photograph it for you yes
