they have an email account on my courier (0.47) server and this is for a
virtual domain so it authenticates against userdb using
courier-authdaemon - and they can download emails from the server no
problem.

basic stuff i know - but...
esmtpd - what does it do? does it receive email from all sources or forward
from email clients?
i.e is it the front end for the main courier email server - receiving email
for that server from anywhere on the internet - or is it a running smtp type
server which
if esmtpd receives email for a server - and can also act as a smtp server for
email clients - how does it handle authentication and accepted domains etc.
i.e. - if the esmtpdacceptmailfor file contains a limited list of domains -
how does it allow client email programs to send email to anywhere

Try `man courier` (or http://courier-mta.org/) which explains most

After installing Courier, you can use man esmtproutes or just man courier
