I have just found out that the bug does not occur any more when I
switch off desktop effects completely!
My GPU is a GeForce4 MX. According to NVidia, I should install the
legacy driver instad, but this seems to be impossible:

I just tested it with acroread 7 on Ubuntu Gutsy and it works just
fine for me. I use these forms all the time with Windows and Linux
and they always work with Acroread and the professional version of
Acrobat, but never with evince.
The form is deliberately simple--there is an input box with a

I, too, can confirm this bug on Ubuntu Hardy Heron (64-bit version).
When attempting to play any streaming video, Totem will only buffer a
few seconds at a time, despite configuring the correct connection speed.
There appears to be no way to alter this behavior. Any of the videos
linked above

I can confirm that when I use fast user switch with the new drivers in
hardy-proposed I don't get the white screen where I can't see the login
window. I get a black screen with a login window visible, as expected.
