I've been taught to use (once the device is well connected and
recognised):
adb shell stop b2g
and of course, at the end :
adb shell start b2g

That's really odd. Did you file a bug for that? If not, please do, and share
the link!
Is this happening in the addon installation process or regular file download?
If the first, did you try right click - save as to download the XPI and open
it in Fx after the download has finished?
Thanks for the excellent instructions, Chris! The same instructions
worked for me on a previously-rooted Nexus 4; I just had to modify the
installer script to reference mako instead of hammerhead. Although
the Wi-Fi doesn't seem to work in FxOS, but I suspect that's a separate
issue.

Turns out my WiFi problems were in fact because of missing symlinks in
the image. I wrote a script to find all the symlinks that are supposed
to get created and regenerate the updater-script file correctly. I
posted my modifications to

Tl;dr: we're ready to lock down 3rd party repo revisions on previous
b2g-manifest branches (v2.0, v1.4, v1.3t, v1.3). However, there are two
problematic repos.
git://codeaurora.org/platform/external/icu4c
refs/tags/android-4.0.4_r1.2 doesn't exist; it looks like this stems
from bug 924359,
The problem seems fixed by disable lock screen in setting.
So dropping adb is caused by lock screen but not screen off.
