When we try to submiit an item to a collection in 1.6 version, we follow all
the steps and everything seems to be ok but after the proccess is complete,
no item appears in the collection. Checking the logs I've found this:

We used to have that kind of errors and in pur case were cause for an error
in the code of the University of Minho Stats add-on that we had installed
previously. The problem dissapeared when we completely removed that
add-on... Maybe this post can help you...
