Turn up logging to Maximum and the logs will show you the users/contacts/etc it is having issues with. Clean those issues up and regen the OAB. You will then be fine. Just had this issue and the customer had 112 contacts with no X.400 addresses. Once that was resolved the OAB regened just fine
