At the moment, you can’t. Or at least, VoiceOver does not tell you the state whether the folder is checked or unchecked. Please report it to the dropbox support so perhaps they will actually do something about it. They definitely are aware. That has been broken for quite awhile. > On Jun 8,

For anyone that's interested, it seems Cheryl's method works for the root directories of one's Dropbox folder, but not sub-directories. At least that seems to be how it's working for me. Unchecking sub-directories somehow un-checks the main directories, which results in me needing to re-check
