Hmmm... good question. There is no OLA implementation. You will have to use
SLAs and have agent accounts (as customers) to assign these to.

Probably not. OTRS is not just a web application, it needs root access,
cron, access to the Apache configuration and a whole bunch of perl
libraries. It's best to install it on a dedicated system that you have full
control over.

Like Shawn said, check your logs.
When you create a "note external" it will send an email to the customer, if
it's not doing that then the only place to see why is in the log files.
