The PostGIS table must have an integer (int4) primary key column and a

The Georefencer plugin is useful for 2D transformation (Helmert 4
parameter, affine, etc.). AFAIK 7 parameter 3D transformation is not
available in QGIS.
You can use cs2cs (proj4) for known projections to transfor from one SRS
to another.

Did you enable on the fly projection in the project properties window?
QGIS cannot reproject rasters on the fly, so you must set your project
projection to the same projection of your raster layer. QGIS will
reproject vectors on the fly.
