You do not have to buy a custom to get a lugged Riv frame, There are still non-TIGed production frames - Atlantis, etc. The steel 3 speed Shimano is the only hub that I have ever had that broke spokes, but maybe it was the bike (Trek Belville). The Alfines make a lot of noise (even brand new)

I've never had to use the Wheels dropout saver, but it seems a perfectly reasonable solution. The only thing you're drilling out are threads that are so damaged as to be worthless. Another, possibly more invasive, solution is to have someone braze some brass into the threads, and tap threads
