So I think I figured it out... the original EXPLAIN was showing it using
Groups2 index. I forced a query against the Groups1 index, and the results
came back in under 1 second vs 11-15 seconds before. I dropped the Groups2

I just looked at the server log. It is passing the username and
password as part of the URL in CLEAR TEXT!
This can't work with external authentication - by which I mean,
the authentication is done by the apache server, not by RT.
It uses the REMOTE_USER environment variable, not a URL.
