I think if you are using a sip trunk, you should "Enable For Internal
calls" only and test. I found it worked best this way. Then, yes
enabling a different ring tone for the line(s) (at the phone, not in
sipxconfig) which works on outside calls, seems to work well (for me).

Yes, that's exactly what I did.
I have two gateways connected to sipXecs. One is an AudioCodes and it
is on the same subnet. I now understand that I should not expect this
feature to work with calls that come through that gateway based on Jeff
Ferrara's comment.

With multiple lines on a phone, go into the phone, click on the inbound line
and then the registration menu item to the left, then change the label and
ring typ

PSTN / ISDN gateway (audiocodes / patton etc) , if the gateway resides
on the same network as sipX, there is no method for differentiation
between an internal and external call - IMO this limits the usefulness
