casue performance issues if the Squid machine has to do a reverse
lookup on every request, the best way is to implement some sort of
authentication or identification is to get a username into the log

I'm running the latest Icap and Squid sources and this problem appears.
The problem definitely seems to be within the squid code.
I've managed to track it far enough to find that in httpReadReply(), it
