launcher intermittently fails to appear when a window is maximised and

I just booted up, started a Terminal and maximised it. I was unable to
display the launcher by pushing the mouse cursor against the left edge
of the screen, and had to hit the Super key to get it visible.
Opening another maximised window (Firefox) seemed to correct this,

While sqlitebrowser is running, an empty slot appears on the launcher
for it. This slot can be clicked to bring the sqlitebrowser window to
the foreground. However, the slot shows no icon.
This problem also occurs in the Alt+Tab list.
