Scott,
If you go into /var/www and ls -la you'll see what the group and user
settings are. I'm pretty sure that your /srv directory and it's contents
needs to be the same for apache to access it.

Except upgrading from 12.04 to 14.04 is one shot. You don't have to go
through the various interim releases. One thing to remember, you have to
uninstall nvidia graphic drivers prior to the upgrade.

After doing the upgrade I was also having issues with FF but they were
