That's not actually the relevant error-state, but an earlier non-fatal
one. If you look in dmesg you can see when that first error-state is
generated prior to the hang and clear it with echo
/sys/kernel/debug/dri/0/i915_error_state and then attach the error for
the fatal hang.

You're right, an error occurred the previous night, when the computer
was unattended, and I didn't notice (dmesg output below). I rebooted
anyway so the error state is now lost.
Are you suggesting that I could have reset the i915_error_state and
catting it would then have printed the subsequent

Thank you for taking the time to report this bug and helping to make
Ubuntu better. This particular bug has already been reported and is a
duplicate of bug 1025488, so it is being marked as such. Please
The problem did not occur in releases prior to raring (just checked
12.10 from a live-usb and there is only one monitor output). Raring is
special: When running unity I didn't noticed the bug because the second
imaginary monitor is disabled by default (but still present in the
system settings).
It's getting even funnier. By disabling the radeon graphics card there
are three monitor outputs now! Maybe it is a problem with hybrid
graphics? Screenshot:
