Yes your HD has failed :( do you have Applecare on it still?
You will need a new HD and as Joe said they maybe a chance to recover some data
if you but the drive in an external case. But of course the most important
thing is to get a new HD :)

Me again! If your or any ones HD is defective do not discard it without
disabling it. There are a number of ways of doing this. If you have access
to an electric drill, drill a hole about 1 inch (2.5 cm) off the center of
the HD. This will ensure that no data can or will be recoverable.
