I'd support Ross in this.
I recently did the same thing with a client of
mine (not web design) even though it meant taking
a significant hit on cash flow - the hassle I was
getting just wasn't worth it. The funny part was,
within a week or so I had three calls from other
consultants I know

If they're insistent on using the photo then why not try the Windows
Vista-style design? I think Microsoft have hired some half-decent designers
as their Vista site is actually really nice visually.

I'm going to add my 2 cents worth on one point though in general, I agree
with Deb and Ross. If you do continue to work with them, insist on a better
picture. The out of focus one they've given you looks horrid as a web page
and it will totally detract from anything else you do,

Agreed, I rarely have to fire a client but have done so on a couple of
occasions. Typically I screen the real PIA people out before they become
clients. As I told one person I'd rather be a greeter at Wal-Mart than work
with some people.
