Commercial insertion is contractually possible on some of the national feeds
(like ESPN) but it isn't something we are doing right now.

Just a clarification, it is $2500 up front and $1000 month minimum. We are
however flexible on the term, the 5 year deal is really only firm on the MDUs
where there is a network buildout.

to go.
by the way, it's the content providers that are requiring the layer 2

They just can't stop marketing to your customers. Once you give them Dish
Satellite, they will bundle in other services in the bills and continually
blast them with commercials on all the channels they serve, including gift
card offers, etc.
