I'll take a stab here.
This is all surmising on my behalf based not in fact but only in my own
paranoia of Microsoft.
My guess is that MSN is attempting to pass things back to your browser and
probably is being passed stuff (do you like these technical terms) by
Explorer. I guess you are using Explorer? And the firewall is preventing
said passing. I'd be interested to see if any firewall router would cause the
problems. Your references to "Works fine with cable, worked fine with ADSL,
worked fine with bellsouth.net dialup" I assume that means with "no STN " in
between these connections when attempting to connect to MSN? Does you STN
dial up bellsouth.net, or connect to the ADSL and the cable? Or do you mean
that once connected with STN to these ISP's you are able to http to msn.com?
I'd also be interested to see if the same things happen with Netscape on the
direct connect calling to MSN.
So, rather than just ramble on, I decided to do a search on www.dogpile.com
"CHAP authentification error" shows up 0 on a dogpile.com search.
but
chap authentification error
with no quotes at www.dogpile.com got some stuff you might want to read.
It's a login protocol and the NT Web Server at Microsoft (you mean they don't
use Linux) is giving you a time of it. Check out these sites for more help.
In a nutshell, I'm not a Linux guy so I'd be in the same boat you're in. But
then I'd dump MSN and get another ISP. I'm not religious.

First you need to know what authentification method your ISP uses. This can be
PAP or CHAP or just a plain-text password. Then you need to tell kppp to use
that authentification method. I'm not familiar with kppp, so look at the
documentation. :-)
