I remember back in '99 when April Escamilla wrote:

Ah! I was wondering why it was dumping you into the root shell..
What you need to do is run fsck /
This will probably ask you if you want to fix some erros in your
filesystem. I don't think you have much choice but to say yes.
When you get done with fsck, you will be able to log out of
the root shell (exit, ^D) and the init should proceed normally.
