As far I know, the UAC doesn't know what will happen.

For instance, RFC 3841 section 9.1 indicates should instead of must.

"When the caller specifies a directive, the server SHOULD honor that
directive."

If they chose to honor the "no-fork" directive, I'm not positive how the
working group intended "proxy to only a single address" to be interpreted.
I assume that it would allow call forwarding unconditional/busy.  Similarly
I'm not sure how the working group intended to apply "no-fork" to generation
of 181 by the forwarding proxy when it would create an early dialog.
