Enabling Music on Hold causes us to immediately be unable to retrieve a
call that was placed on hold (the call disconnects as soon as we hit
Resume).

Changing RTP Keepalive Method from "None" to "Empty Packet" also didn't
help (no change in symptoms.)

The call is terminated by the BYE at frame 316, which is indeed 1 minute
after the call is put on hold.  But that BYE comes from what appears to
be the outside caller, "6163074699" <sip:6163074...@voipmich.com>.  So
you really need to ask voipmich.com.  My guess is that voipmich did not
receive any RTP for 1 minute, and so decided to terminate your call.
IIRC, if you configure sipXbridge to provide music on hold, you can
ensure that the outside caller always gets RTP.
