I am afraid it is very ad-hoc altogether, a combination of World Machine, Vue, 
Zbrush and some clever tricks is simply critical if you want to do 
sophisticated terrains and there is close to no literature on the subject.

I did a job recently with 6km by 6km Swiss range of mountains (pretty amazing) 
and resolution to 1m that we did some extra resampling to go down every further 
and the datasets are so huge it is scary.

Looked great, happy with the results, the process without the help of an expert 
would have been a total nightmare so it is on my to-do list of course but 
believe me when I say you are going to spend a ton of time finding out things, 
from formats to workflows…
