I've received quite a few forms now and I think I'm good. Thank you to
all that shared.

BTW, I'm still trying to get a servicer selection guide developed for
our business owners in the city. I have some basic questions already but
am looking for more. Please send any ideas, questions, etc. with regards
to problems you encounter on a frequent basis so I can combine them into
what I've gotten already. This isn't meant to be an all inclusive
listing but to provide some questions requiring honest answers and the
ability for a business owner to understand why one or two contractors
might be priced a bit higher than the low bidder. After reading a few of
the sample questions, you guys will get the picture. Again, any
help/suggestions are welcomed and I'll share the final draft result with
the group.....

For example, some questions may be:

General
- I don't have a copy of the manufacturer's literature or record
drawings (as-builts) as required by NFPA 25 and the local AHJ. Do you
really need them and how do you suggest we solve this deficiency?
- Will your inspectors be verifying that my current storage conditions
are satisfactory for the current sprinkler system so I don't get issued
a Correction Notice by the local inspectors for having inadequate
protection as part of this service/inspection program?

Dry Standpipes
- Does my building/facility have dry standpipes?
- How does your company test dry standpipes? Where will the water source
come from and what will the flow rate need to be?
- Will I have to pay to rent a water meter or is it included in the
price quote?
- Does your basic inspection quote include the rental fee of a large
pump or the hiring of off-duty firefighter to achieve the proper water
flow rate at roof level of our high-rise building?

Foam Systems
- Please describe the type of foam system I have and the required
testing needed (hydrostatic testing of tanks, pump
- Will your technician be taking a sample of foam during a full flow
test to verify the concentration?
- What method will be used to verify the concentration?
- What environmental issues might arise with the disposal of foam during
the testing and how can those be mitigated to prevent problems with the
water/environmental agencies?

Again, this will be a guide our department can give to local businesses
so that they don't get "stung" after we reject a confidence test report
for being incomplete and have to pay extra to have everything that is
required done.

I have both certificates as WORD templates and have already sent them to
David. If anyone else needs them, let me know directly.
