I've had some of the same issues, and here's what has worked for me. How many entries are typically in the temp-ok group? I realize it goes up and down, but I'd bet it's normally a couple hundred? A thousand?
Set up your scripts so that every time you are making a change to temp-ok they delete the .db files in temp-ok. Make your changes directly to temp-ok/domains and temp-ok/urls... if it's not in there and you need it, add it. If it is in there and you want it out, delete it. Then do your 'squid -k reconfigure'. squidGuard will start from the text files in temp-ok, but the number of entries is so low that you'll never notice the difference. Any regular maintenance scripts that you have may build new .db files in temp-ok, but that's alright, because you'll delete them when you make changes.
