Can you post your squidguard.conf file ? I Have my sites working blocking sites 
but don't know how to setup the whitelist. I don't use any ip based groups etc 
so therefore only have the default acl.

It's strange, i have squid-2.5.stable12 and squidguard-1.2.0 compiled by
source on Whitebox linux. When i make any change at squidguard's db file,
then i do "squidguard -C all" and after "squid -k reconfigure" and then the
 access lists are updated. Just tried today and it worked. I modified the db
 file to allow a new site. Maybe that is different trying to remove access
to
 a site previosly allowed ?
Anyway, can you copy and paste the "reload" section of your
 /etc/init.d/squid init script, so we can see what it does in the background
?
