Issuing a reconfigure is essential for us, as out kit is so old
it takes ~4 minutes to index the pr0n lists alone.

The time reconfigure didn't work for me was when access
rights were wrong on the db  files - this happened depending
on who edited the lists and ran the updates, etc.  It was
fixed by writing a rebuild script that sets the correct
ownership/permissions prior to running squid -k reconfigure.

I think there was also a problwm with using the wrong version
of BDB, where squid would work fine with native block lists,
but once you created the .db files, it wouldn;t block anything.
