 if you get long pause - send my greetings to packet dropping firewall and
try connecting to your provider's smtp server.
 if exim drops the connection - check exim logs (/var/log/something).
 if you get connected - try sending message.

check firewall rules in qmail and exim servers.

check your Terms of Service agreement with your ISP. In some cases you are
not allowed to run servers and providers block outgoing SMTP connections.

go to http://www.dnsstuff.com/ and check your qmail server's ip address in
first form in middle column. some servers can block you, if you are listed
in some blocklists (sorbs,njabl,maps).
