A few of us have had the same problem but using OS 9. The solution
was to replace the Apple CD/DVD extension with the earlier version
1.4. So I am wondering what version you have with OS 8.6?

>I just installed a Sony CRX140E IDE CDRW 8x 4x 32x in my 4000/
>Metabox 320Mhz G3/ 128MB RAM (240 with Ram Doubler)/ OS 8.6. Every so
>often I am getting a flag telling me the CDRW can't read a drive, and
>it asks me to eject or initialize, but this is only happening when
>there is NO CD in the drive.

>Other than this the drive works wonderfully, it's even bootable, this
>is happening using Apples CD/DVD Driver. Has anybody ever seen
