My take:

Where the access key is used on a transport object such as a highway, a railway then it refers to the mode of transport. Where it is used on an object that is not a transport object such as a toilet then it refers to the human access to the object. The OSM wiki is not clear no the topic being biased to transportation needs.

If there is a need to specify the mode of transportation to an non transport object then a transport object can be connected to the non transport object. Example a footpath going the entry door of a library could have access=yes, bicycle=dismount while the library could have access=yes, bicycle=no ... hopefully there is a bicycle parking node nearby. Wheelchairs use wheelchair=yes/no for access determination.
