You can probably guess what I'm going to say. <g> As I have said elsewhere, I think TB's interface is in great need of a facelift. Not just to be "prettier" (although it wouldn't do any harm) but to be easier and more productive to use. Good (interface) design is a combination of function and appeal. Sadly, TB suffers in both areas. Although TB has a great many excellent /capabilities/ (for want of a better word), many of them are both difficult to use and inelegantly implemented.
