  As I understand it, tex is being used throughout, so that latex and its
packages don't come into play.  If etex
can be used with plain tex, I presume Don has already looked after that in
pmx.

   All the notes are scrunched up together, so that one cannot disinguish
one note from another.
All the notes of the all the bars are together in about an inch or so.
