The Apache CouchDB is what Cloudant is providing, You don't need both. If you are intending to host your own, then you wanted Apache CouchDB (I will assume this is what you wanted).

You want to set your admin name and admin password, this is your credetials as a CouchDB administrator/owner. Normally this would be a separate account from the one you use for your NoteSelf but it could be the same. for IP, it depends on what you want... IF you will only ever be using this from the computer that you install Apache CouchDB on, then put in "127.0.0.1", otherwise put in "0.0.0.0".
