I'm probably shooting myself in the foot - and quite possibly showing my ignorance - but in reference to:

"Received: by simscan 1.1.0 ppid: 26655, pid: 26656, t: 0.4758s
    scanners: attach: 1.1.0 clamav: 0.83/m:31/d:867 spam: 3.0.2"

...I was missing this too, until I started going back through the toaster and found two mistakes I had made.

Under Vpopmail -

echo '127.:allow,RELAYCLIENT=""' > ~vpopmail/etc/tcp.smtp
(cd ~vpopmail/etc ; tcprules tcp.smtp.cdb tcp.smtp.tmp < tcp.smtp)

and under simscan -

echo ':allow,QMAILQUEUE="/var/qmail/bin/simscan"' >> ~vpopmail/etc/tcp.smtp

...I used the ~'s [`] instead of the " 's [']

After I corrected these mistakes, I found that simscan and clamav were working properly.

If this doesn't help, then please forget I ever posted this embarrasing work...
