The installation process has a mandatory reboot step. If you skipped that
step something may have come out of sync. However, this does not explain all
the trouble you are going through.

The icons do not propagate between machines. This is expected.

Not as far as I know.
But since you see those icons even with TortoiseHg and TortoiseOverlays
removed from the system - and in particular with no "*tortoise*" string in
your registry, there must be some cache somewhere.
