What we do (although not automatic) is our system checks every hour
for any documents in a specific directory (SMB mounted on Unix) 
(placed there by the scanning process - running XP).

When it discovers documents, it assigns it a "scan number" and moves
the document into a holding directory, the path to the holding directory
and document name (assigned by the scanning process) is saved along with
the scan number. (this is the automated part).

An email is sent to someone that there are documents waiting to reviewed.

>From this point, we have a webpage (combination javascript/cgi link to
  a universe program) that will pull the saved document and display it
  (uses ftp to pull the image from the document machine to webserver machine
  and saves using a meta filename, which is used in the dynamic HTML page).

  This webpage allows for the document to turned (if scanned upside down
  or the original document was faxed in sideways). It uses the IE image
  manipulation styling and saves the code for how to display (how many turns)
  so future displays of the document will always display properly.

  Any information at this point is manually transferred from the scanned
  image to second table with input fields

Future addons - Using the number of turns codes, the document will be placed in
  any of 4 directories (90, 180, 270, or 0 degrees turn). Then an image program
  will be run in batch mode to "turn" each image x degrees and resave depending
  on which directory it's in, thus the end result will be all images will be in
  the same orientation, eliminating the need to re-orient the image when 
displayed.

We found OCR to be to unreliable in trusting the data that it strips out. There
are some things, a human is still required for.
