Just for the record, I don't disagree with "test according to the test plan even if other testing also works". It was an oversight on my part that I didn't do that, mainly because I had helped find the solution to the bug with Erich and so was very familiar with what the issue was and how to ensure that it was no longer present. It's not customary to require mentioning the exact package version, but I wouldn't be opposed to a new policy requiring that (and wouldn't be very surprised if one exists now that I simply wasn't informed of).
