If there was any hint of slowdown before the 6.0.0 update, then I would suspect the VM a bit more, but in this case I simply shut down a happily running Win7 guest of 5.2.22, updated virtualbox and virtualbox-ext-oracle packages to 6.0.0 on Arch host, (rebooted) and then launched the Win7 guest and the problem appeared. I'll strace and see what turns up. This is one of the joys of having the server live on the bleeding edge with Arch... While the vbox kernel modules built without issue, that doesn't rule out some strange versioning issue with something well down the line. Will report back when I have more.
