When I follow
your steps, I can get the results you want.  I noticed that I was
getting some weird
results when I tried replacing the selected text with the '%' key, which led
me to check my mappings.
Sure enough, % in visual mode is mapped by a plugin I've installed.

Is your Q key mapped to something?
