To the best of my knowledge there is no standard "pattern" for these  
adapters, instructions were usually included to assist in configuring  
them.  That said, here is what I found with a quick search:

MAC/IBM ADAPTER: 15DM/HF-10P
DB-15Male/HD-15Female, Mold type, with 10 pos Dip switch adjusting  
the signal
between Macintosh Computer & IBM Monitor. Support 6 kinds different

MODE 4 is SYNC ON H DB-15M pin 15 conn HD-15F pin 13

MODE 6 is SYNC ON GREEN SYNC ON GREEN
