A great tool I used for my Mac SE is called Lido 7.
http://www.euronet.nl/users/ernstoud/lido7.html
Not recommended for anything above 7.5.3, so if you are using on
anything higher, I recommend MicroNetUtility.
http://www.euronet.nl/users/ernstoud/bnchmrks.html is a great site for
others if neither of those two work. DL links are also there for both
Lido7 and MNU.
All of these utilities can format any Mac HD along the SCSI chain.

If you need a boot disk, you can either download a sys6 or sys7 disk,
or hold down CMD-OPT-X-O on boot, and that could boot into a ROM-
stored read-only version of 6.0.3. Undocumented feature present on
some (if not all) mac classics.

If you need images for floppies, try here:

or here http://www.earlymacintosh.org/disk_images.html

Hope that helps!
