DA-15 (Apple Video) to DE-15 (VGA) cables won't always work as they are set up 
for specific monitor types.  You may need an adapter with switches that you can 
configure for the specific monitor / computer setup.

 

It may also be that your monitor doesn't support sync-on-green.  Early Macs 
carried the video sync signal on the green video line.  Mac monitors extract 
the sync.  SOME VGA monitors do it, many don't.  While there were adapters to 
extract the sync they were uncommon and quite rare now.  If it is sync-on-green 
you'll need a different monitor.  But finding a VGA monitor that supports 
sync-on-green is problematic, few advertise it or spec it in the manual.  I 
have found some modern LCD monitors support it.
