I do not have that bug of silence for up to 90 seconds or so, very rarely does 
any of my iOS devices go quiet like that... and even when they do, its for no 
more than say 5 seconds max.

do you regularly, and I mean regularly, like every night, empty your app 
switcher, and restart your phone... I do this religiously every night, clear 
the switcher and restart the phone.

also do this on my iPad, perhaps you are experiencing something which is 
setting or app specific to your device.

For all the great accessibility Apple's done with the phone, this is a major
bug one would figure would be addressed by now in some fashion. Then again,
who knows, maybe it's just not that easy to fix. 

I recently got an iPad 2 and for the first week or so, I hadn't experienced
that issue and started getting all jazzed that maybe it was just an iPhone
thing, but of course, reality had to step in and and yesterday, I
experienced the Voiceover going silent bug twice in about an hour's time.

Oh well, it was fun while it lasted.

It doesn't really sound like this release addressed much. I would not be
surprised if there are few or no fixes for anything accessibility.
They may have made a quick fix for the app launching on unlock since people
made such a huge deal out of that small avoidable not really a problem
issue, but I wish they would fix the speech locking up over that one. It is
really strange though that they put out this release to fix what appear to
be such minor issues and they let huge issues hang out there for 6 or 9
months until some planned release and at least claim to have addressed them.
Purely speculation here of course on all of this, but I am betting there was
some change in this release that Apple has some motivation to make and it is
not written anywhere.
Something like a jailbreak whole patched or something.
