It is there so updates are not installed automatically as the setting implies.
Basically the behaviour with automatic updates turned off is the same as before 
this option was introduced in iOS 12 and that is that the update downloads, but 
it won't update unless you go into Settings > General > Software Update and tap 
install, then enter your PIN and agree to the terms and conditions.
You have to remember that Apple still tried to get as many users onto the 
newest version of iOS as possible, but from what I read online most people have 
little trouble to not update if they turn automatic updates off and delete the 
update from their phone in case has already downloaded. I guess eventually it 
may redownload, but if you don't want to take any chances for that to happen 
you could always turn off WiFi over night when many people plug  in their 
phones since an iOS update at this point will definitely not download over 
cellular ddata.

Thank you for this, as I do and have always had auto download turned off.  I 
think rather this feature is turned on or off, the download will still 
download, but if we don't want the update on our phones, we have the option to 
install by accepting the license agreement or not.  This is the way it works on 
my phone.  Although, I'm just not for sure why we have the auto software 
option, if the update still downloads.

If you have automatic updates turned on, it will download to your phon e 
whether you want it or not. The best way to keep it from downloading is to turn 
this setting off in settings, general and software updates.
