It is there so updates are not installed automatically as the setting implies. Basically the behaviour with automatic updates turned off is the same as before this option was introduced in iOS 12 and that is that the update downloads, but it won't update unless you go into Settings > General > Software Update and tap install, then enter your PIN and agree to the terms and conditions. You have to remember that Apple still tried to get as many users onto the newest version of iOS as possible, but from what I read online most people have little trouble to not update if they turn automatic updates off and delete the update from their phone in case has already downloaded. I guess eventually it may redownload, but if you don't want to take any chances for that to happen you could always turn off WiFi over night when many people plug in their phones since an iOS update at this point will definitely not download over cellular ddata.
