Janet, I also have Settings, General, Software Update, Automatic Updates turned off but as you said, iOS 13.x updates have still downloaded to my iPhone 8 numerous times. When I get a popup asking me if I'd like to install the downloaded iOS, I always select later and then I'm prompted to enter my personal ID after which I'm prompted to agree to terms and conditions. I always disagree in the bottom left corner of the screen and the update is not installed. I then immediately go to Settings, General iPhone Storage, iOS 13.x and double tap on the downloaded iOS in the list, select delete, and remove it from my device. A day or two later, the download happens again and I repeat the cycle.
I'm not interested in leaving iOS 12.4.1 until iOS 13.2 is officially released and they've hopefully fixed the bug with Voiceover and third party software keyboards.
