What I find interesting about that rotor item, now that you reminded me it was there, it was selected on my rotor items, but it does not show up in the rotor unless you are using an app that does not use the standard text entry. But in iOS 14.4.x apple fixed it so you just ignore VoiceOver when it says Direct Touch Area and keep typing. I have never found a need to turn the rotor when VoiceOver Says Direct Touch area. Things just work.
